We are located behind (east) Fashion Place Mall in Murray. The name of our building is “Cottonwood Plaza Medical Center”. The building is marked 6095 in large black numbers on the top right of the building. We are in a multi-physician complex, northeast of Dillard’s at Fashion Place Mall. Our building is NOT a “University of Utah Health Network” building.

I-15 going south bound

1. Take 5300 South exit
2. Turn left onto 5300 South, continue to State St.
3. Turn right onto State St. and continue until you reach 6100 South
4. Turn left onto 6100 South until you reach Fashion Blvd.
5. Turn left at the green arrow (you can only make a left or right)
6. Immediately get in the right hand lane and make a right at the first driveway to the Cottonwood Plaza Medical Center parking lot.

I-15 going north bound

1. Take 7200 South exit
2. Turn right onto 7200 South, continue to State St.
3. Turn left onto State St. and continue until you reach 6100 South
4. Turn right onto 6100 South until you reach Fashion Blvd.
5. Turn left at the green arrow (you can only make a left or right)
6. Immediately get in the right hand lane and make a right at the first driveway to the Cottonwood Plaza Medical Center parking lot.

I-215 from east south bound loop

1. Take 280 East exit. Continue going straight, this takes you directly onto Fashion Blvd.
2. Stay on Fashion Blvd. At the signal light, turn right at the first driveway to the Cottonwood Plaza Medical Center parking lot.

I-215 from west south bound loop

1. Take State St. exit.
2. Turn left onto State St. and continue to 6100 South
3. Turn right onto 6100 South until you reach Fashion Blvd.
4. Turn left at the green arrow (you can only make a left or right)
5. Immediately get in the right hand lane and make a right at the first driveway to the Cottonwood Plaza Medical Center parking lot.
