glad to be a part of a great organization, always striving to be better, if that's possible.

September 06, 2016

Dr. Jack is always kind and caring; he explains things well and leaves us with the feeling that he truly cares about us and that he considers all factors. He is so complete in his analysis and recommendations, we feel very safe in following his advice.

September 03, 2016

July 16, 2016

dr. Jack didn't act rushed or put out and he did a really great job of addressing my concerns . it was refreshing to have an experience with the Doctor Who didn't act like he had to rush out in five minutes to see 45 other patients

June 28, 2016

Good

June 27, 2016

Jackie Lunt was the PA and she was excellent - very thorough and kind and courteous while being concise and informative. She was quite wonderful. Dr. Jack, as always, was exceptional - and very patient with me - a somewhat reluctant and uninformed patient.

May 17, 2016

I am incredibly fortunate to have Dr. Jack as my cardiologist. Here are just a few of the many things that I love about Dr. Jack. I never feel rushed when I am meeting with him. He is such a quick study...he listens intently then provides thoughtful and valuable feedback. Dr. Jack thinks outside the box and provides/suggests alternative solutions to problems that may not be cardiovascular in nature. He always ends each session with a review and summary of what was discussed and the course of action on how to proceed. The U of U Hospital and Clinics is so fortunate to have someone of Dr. Jack's caliber on staff, as are his patients.

May 16, 2016

Dr. Jack is just the Best ... you can't improve on that!!

May 13, 2016

he was great & down to earth in his discussions, etc. he seemed smart & knew what he was talking about. he listened to what I had to say. I appreciate that.

March 19, 2016

Doctor was unable to follow through with commitment to provide warfarin home monitoring equipment. Doctor stressed the importance for me to see my Cardiologist, Dr. Renner ASAP then instructed his staff to get an appointment set for me. After waiting one week with no response from Cardiology I got the appointment set through my PCP.

March 14, 2016

Dr Jack is excellent, very caring very knowledgeable

January 29, 2016

The combination of this PA/Doctor team is without comparison. A survey cannot begin to explain my appreciation for, and confidence in this level of care and caring. True professionals in whom you need to have great pride and respect!

January 18, 2016

He was familiar with my history and very informative. seemed interested in us.

January 18, 2016

I learned that I have a serous heart condition and was in shock. So I could not think of any questions. A little more explaining would have been very helpful. I phoned the next day to ask questions.

December 06, 2015

very impressed

December 05, 2015

I may not have asked enough questions due to being so tired. I hope to get more answers as to how I can get completely well during my follow-up appointment.

September 13, 2015

I have the utmost confidence in Dr. Jack's ability to oversee my care. He is incredibly knowledgable and he explains things in ways that are easy to understand. I have never once felt rushed during an appointment with him. I wish I could clone him and send his clone back East to care for my dad, who desperately needs the kind of cardiac care that Dr. Jack provides.

September 10, 2015

I have complete confidence in Dr. "Jack". I feel he is very knowledgeable and understanding of my situation.

September 05, 2015

I told the doctor at the beginning of my appointment that I had a B.S. in Nursing, so we were able to talk to each other as professionals. The doctor respected my input and I respected his opinion about my condition.

September 05, 2015

Doctor Morshedzadeh is not only intelligent and engaged, but an excellent communicator and teacher. His Socratic Method is very helpful.

August 07, 2015

all good.

May 10, 2015

I couldn't ask for a better doctor to oversee my cardiac needs than Dr. Jack. He is an excellent listener, and a quick study. It is not unusual for him to ask questions about medications I am on that have been prescribed by other physicians in other specialty areas to make sure they don't have negative implications on my condition for which he is treating me. I wouldn't hesitate to recommend Dr. Jack to others.

March 02, 2015

Couldn't of been treated better.

March 01, 2015

I have been pleased with Dr. Jack and have already recommended him to other people. His strengths are his careful, thoughtful, and easy-to-understand explanations and comments on my health condition. He is kind and seems to have all the time in the world to spend with me.

January 04, 2015

He seemed very caring and understanding.

January 02, 2015

Often I feel as though I am the only patient in the clinic. Everyone is professional, efficient, caring and attentive.

November 30, 2014

Dr.Jack is very efficient. He cares a patient. He told me what he could help and what he could not help me. I liked that.

November 01, 2014

Excellent

July 17, 2014

This is my first visit with Dr. Jack. I miss Dr. Green but I have full confidence in Dr. Jack

July 15, 2014

Dr Morshedzadah answered most of my questions before I could ask them.

July 14, 2014

Dr. Jack is one of the best care providers that I have. Very concerned, detailed, and professional.

June 05, 2014

I always feel like Dr. Jack treats me as a participant in my own care. He listens, he explains⿦ and he seems to anticipate some of my questions.

June 02, 2014

Dr. Jack is always a good listener, easy to talk to, and makes sure I understand what I need to know about my medical problems.

May 31, 2014

As a physician I was very impressed with Dr. Morshedzadeh's broad knowledge, general acumen, sharp focus on my issues, and plan for going forward. I previously dropped using the CV service at the U because I was disappointed in how I was treated and what I considered inadequate medical management. I just returned because I heard that I would feed differently with Dr. Morshedzadeh, and that indeed proved to be the case. I am very glad that I came back to the U as his patient.

May 03, 2014

Dr Jack was excellent to talk with.

April 28, 2014

Always good service by U

April 25, 2014

Dr. Jack deserves a 10+ when it comes to the confidence I have in him. A rating of 5 doesn't do him justice. He loves a good challenge and he meets it head on.

April 18, 2014

Dr.Jack carefully explained my diagnosis and answered my questions. He told me how my medicine works.this helped a lot so i could have confidence in my treatment plan.

March 30, 2014

compared to my last university experience this was great. Dr. Jack listened to my concerns and answered my questions.

March 27, 2014

Both NP and Cardiologist were engaged and reassuring

March 21, 2014

The doctor wants me to try a new medication to regulate my heart beats. At the time, he didn't make it clear to me that he thought the problem might be due to another med. that I have to take for another condition I have. I called the clinic and discussed this with his PA, now I know what he's trying to correct.

January 16, 2014

Dr. Jack is an amazing Cardiollogyst. He always comes in and explains everything very clearly. He involves his patients in making all decisions. I would highly recommend him. He is very knowledgeable.

December 22, 2013

I am very skeptical about working with this provider. His PA seems to care about me more.

December 16, 2013

Julie Savage stayed with me - even after the clinic had closed on a Friday evening - to make certain I got the medications that were prescribed. She even went to the pharmacy with me and gave me her cell number in case I had any questions or concerns. This is amazing to me. She explained everything to me, included me in decisions about my care and was THERE for me. She is remarkable and an outstanding member of the cardiology team.

December 08, 2013

I have complete confidence in all who take care of me in this facility.

October 21, 2013

I was very fortunate to get Dr. Jack as my cardiologist after my previous cardiologist passed away. Dr. Jack is incredibly knowledgable and a real problem solver. He asks lots of questions to make sure he understands what I am dealing with and so that his treatment plan is appropriate for my situation.

Dr Jack is so much better than the first time I saw him.

June 04, 2012

I was going to write a letter of thanks to the Cardiology Department for such positive visits with Dr. Jack and Dr. Akoum in the past. Perhaps this survey will say it.

May 04, 2012

Dr. Jack was great.

February 25, 2012

I was extremely fortunate to have Dr. Jack take over my care. Dr. Jack takes his time with me and asks lots of questions to make sure he understands what it is I am trying to say and that we are both on the same wave length. He is a quick study and excellent at assessing a situation.
