Due to the need for specialized services for people with developmental disabilities all over the state of Utah, HOME is committed to expanding its service locations to better meet the needs of our patients. In August 2009, HOME added an additional site in Ogden where patients can receive their primary care and therapy services. Starting September 1, 2010, we will also be offering psychiatric services in Ogden.
Current HOME patients can call the HOME North Clinic directly for primary care appointments. For psychiatric and therapy services, talk to your case manager at the HOME Program for a referral. New patients interested in being seen in Ogden should follow the same scheduling process for enrolling in HOME as other potential enrollees.

Turn East on 25th Street from Washington Blvd. Turn Right on Kessel Avenue. The entrance to the underground parking is on the south side of the parking structure. The west entrance to Health Clinics of Utah (HOME North Clinic) is next to the parking structure.
