Neurobehavior Clinic: The Neurobehavior Clinic is currently closed to new patients. We apologize for the inconvenience.
HOME Program: If you would like to receive services through the HOME Program, call Medicaid at 801-567-3835 to see if you qualify. If you qualify, they will send you an intake packet. When we receive the completed packet, we will contact you for an appointment. Be aware that demand for the HOME Program is high and it may take some time before we can see you.
