Infectious Diseases - Case Management

The Infectious Disease Clinic (Clinic 1A) case management program consists of on site medical case management to assess a patient's psychosocial needs and coordinate care with the HIV treatment team.

The Clinic 1A case management team provides outreach case management services. The team consists of 6 medical outreach case managers and one lead case manager, all with HIV and social work backgrounds, who provide community-based assessment, monitoring, and evaluation of patients who require more intensive follow-up and assistance. Each case manager works directly with the HIV medical provider to coordinate overall treatment goals and provide continuity of care across the patient's spectrum of medical and psychosocial needs.

One of the main functions of Clinic 1A case management is to educate and link patients to community resources which promote health and self-sufficiency.

Case management services are available to all Clinic 1A patients regardless of level of need and continue as long as the patient is enrolled in medical care.

  1. To provide quality early intervention primary medical care for HIV/AIDS patients in Utah.
  2. To increase awareness about transmission of HIV, and living with and treatment of HIV disease.
  3. To have a formal mechanism for consumer input and involvement in the PCA program.
  4. To ensure that PCA clients are receiving quality treatment and care.
