Billing information for the University Orthopaedic Center and University Hospital.

Your clear understanding of our Financial Policy is important to our professional relationship. Please ask if you have any questions about our fees, Financial Policy, or your responsibility.
As a result of your clinic visit, you may receive four bills:
- One from our billing department for Physician services
- One from the University Hospital, this covers the clinic charges incurred during your visit
- One from the Radiology Department for the x-ray procedure
- One from the Radiologist for reading your x-rays.
Orthopaedic Physician bill includes:
- The doctor's examination or supervision charges for physician procedures
Hospital Billing: (801) 238-6603
Hospital bill includes:
- Fees associated with procedures
- Non-physician personnel
- Exam room and equipment charges
- Medical supplies
How do I identify each bill?
Physician's Bill: account numbers start with the letter "M" followed by several numbers, a dash and then 2 more numbers (i.e. M123456-14).
Hospital Bill: account numbers are 8 or 9 digits long with no letters.
