Self Pay Policy
If we are unable to verify your coverage by an insurance plan, you will be considered a “self-pay” patient. For self-pay patients, It is required that we receive a non-refundable deposit before we will schedule an appointment. This will be used to cover the costs of the appointments.
If you cannot keep your appointment time(s) and need to reschedule within 3 days of the appointment, the deposit will be applied to those appointments. If you cannot keep your appointment time(s), and do not reschedule within 3 days prior to your new patient appointment, you will lose the deposit.If an appointment is already scheduled prior to a change in status to self-pay, the appointment will be cancelled and will not be rescheduled until a deposit is received by our office. This deposit is non-refundable (will not be returned to you).
