Self Pay
A self pay patient is any patient we are unable to verify insurance coverage.
Requirements for self pay patients:
- Non refundable deposit which is approximately 50% of the total before we will schedule an appointment. In addition to your deposit you will be billed for the balance.
- Deposit rate for New Patient Visit
- $500.00 deposit
- $100 of which is non-refundable. This is for cancelled appointments (within 2 days) and not showing for appointments.
- New Patient Visits include:
- Physician
- Physical Therapy
- Behavioral Medicine
- Treatment Plan Visit
- New Patient Orientation (no charge)
The deposit rates for return patient appointments are as follows:
