Q: What Medical Services Will I Receive?
A: Your first visit will include a comprehensive checkup with a primary care doctor who works with teens like you regularly. This checkup helps our health care team get a sense of your general health so they can provide treatment as needed.
Your doctor can provide the following services:
- Annual well checkups
- Management of on-going medical problems
- Birth control
- STD screenings and treatment
- Sports physicals
- Sick visits
Q: Are There Additional Services?
A: We understand that as a teenager, you have unique needs. In addition to primary health care, a team of professionals will look out for your psychological and social needs, and help identify your educational and financial needs to match them with community resources and programs.
Q: How Much Does All of This Cost?
A: The Teen Health Clinic provides affordable health care and accepts most insurance plans. We also offer a sliding-fee scale if you don't have insurance, A sliding-fee scale is based on your income. In addition, we have financial counselors who can help.
Q: Who Will I Meet With?
A: Our primary care providers include pediatricians and nurses who have a special interest in teen health. Social workers are also available.
Q: Do My Parents Need to Come?
A: We welcome you to bring your parents or legal guardian. If they cannot come, we will need you to bring signed permission for us to provide medical care.
- Appointments regarding STDs or pregnancy testing do not require parental permission.
Q: What Else do I Need to Bring?
A: In addition to bringing signed permission for us to provide medical care, please bring the following (if they apply):
- Insurance information
- Immunization record
- List of current medications
Q: Where Will I Go for Appointments?
A: The Teen Health Clinic is located in the South Main Public Health Center at 3690 South Main Street in Salt Lake City, which is conveniently located one block from a TRAX station.
Q: What Other Services are Located in South Main Public Health Center?
A: The Salt Lake Valley Health Department services are located in the same health center, making it easy for you to receive services, including the Women, Infants and Children Supplemental Food Program, Medicaid eligibility counseling and low cost immunizations and laboratory services.
The Teen Mother and Child Program is also located inside the South Main Public Health Center. This program provides medical care for pregnant teens and their babies and works together with the Teen Health Clinic.
