Astellas Recall of Lots of Several Medications Due to Stolen Transport Trailer

Astellas has issued a recall of several lots of some medications. A transport trailer carrying these items was stolen in mid-June and the items have not been recovered. Due to safety concerns should these medications come back into circulation, Astellas is recalling these specific lots of medications. No other lots of these medications are affected by this recall. The affected products and their lot numbers are listed below:

  • Adenosine 3 mg/mL injection (Adenocard): 2 mL syringe (NDC 00469-8234-12), Lot #65220EV; 4 mL syringe (NDC 00469-8234-14), Lot #63334EV; 20 mL vial (NDC 00469-0871-20), Lot #69510DK; 30 mL vial (NDC 00469-0871-30), Lot #73402DK
  • Alefacept injection (Amevive): 15 mg vial (NDC 00469-0021-04), Lot #P27010
  • Amphotericin B liposomal injection (Ambisome): 50 mg vial (NDC 00469-3051-30), Lot #042922AA
  • Conivaptan injection (Vaprisol): 20 mg bag, 10 x 100 mL (NDC 00469-1602-11), Lot #C763474, with or without an alpha suffix identifier (ie, includes Lot #C763474a)
  • Micafungin injection (Mycamine): 50 mg vial (NDC 00469-3250-10), Lot #0500; 100 mg vial (NDC 00469-3211-10), Lot #0600
  • Regadenoson 0.08 mg/mL injection (Lexiscan): 5 mL syringe (NDC 00469-6501-89), Lot #75275EV
  • Tacrolimus capsules (Prograf): 0.5 mg 100 count (NDC 00469-0607-73), Lot #036031 and Lot #036271; 1 mg 100 count unit dose (NDC 00469-0617-11), Lot #034071; 1 mg 100 count (NDC 00469-0617-73), Lot #036551; 5 mg 100 count unit dose (NDC 00469-0657-11), Lot #035081; 5 mg 100 count (NDC 00469-0657-73), Lot #035592
