Correction Notice - Typographical Errors in Pediatric Injectable Drugs, 8th Edition (The Teddy Bear Book)

The American Society of Health-System Pharmacists (ASHP) is notifying practitioners of several typographical errors in the Pediatric Injectable Drugs, 8th edition, also known as The Teddy Bear Book.

The diltiazem monograph on page 146 lists an incorrect infusion rate range for diltiazem continuous infusion in the second line under atrial tachycardias in the Dosage section. The correct continuous infusion rate range is 0.05-0.15 mg/kg/hr.

The dextrose monograph on page 134 lists incorrect age ranges in the hypoglycemia subsection of the Dosage section. The correct age ranges should read: “Term neonates and infants < 6 months” and “Infants ≥ 6 months and children.”

The ceftriaxone monograph on page 92 lists incorrect dosing intervals in the infant and children subsection of the Dosage section. The correct dosing interval for mild to moderate infection should read 50-75 mg/kg (up to 2 g/d) given once daily or divided q 12 h. The dose for severe infections (including meningitis) should read 80-100 mg/kg (up to 4 g/d) given once daily or divided q 12 h.

The rocuronium monograph on page 394 lists an incorrect dose for infants under the Dosage section. The correct dose is 0.5 mg/kg.

The sodium chloride monograph on page 402 lists an incorrect product description. The first line of the Brand names section should read 0.45% saline (1/2 NS) = 77 mEq/L.

Appendix C contains a duplication of page 460, in place of page 461. The omitted content on page 461 can be found in the ASHP correction notice link below.
