Accusure Insulin Syringe Recall
October 28, 2009
Qualitest and FDA issued a recall of all lots of Accusure Insulin Syringes. This is an expansion to the original recall in August 26, 2009 of only two lots (#6JCB1 and #7CPT1) of Accusure Insulin Syringes. The expanded recall includes all syringes distributed between January 2002 and October 2009, regardless of lot number. The affected syringes may have needles which detach from the syringe, potentially allowing the needle to become stuck in the insulin vial, slide inside of the syringe, or remain in the skin after injection. Consumers who have any Accusure insulin syringes should stop using them and contact Qualitest at 1-800-444-4011 for reimbursement.
