Remifentanil Hydrochloride (Ultiva) - Typographical Error in the Handbook on Injectable Drugs

The American Society of Health-System Pharmacists (ASHP) is notifying practitioners of a typographical error in the remifentanil hydrochloride injection (Ultiva) monograph in the Trissel’s Handbook on Injectable Drugs 15th edition.

The Y-site injection compatibility table on page 1411 lists an incorrect concentration for potassium chloride (0.1 mg/mL) in the third column from the left. The correct potassium chloride concentration is 0.1 mEq/mL. The correction notice and an updated compatibility table are posted on ASHP’s website at http://www.ashp.org/Import/NEWS/Corrections/CorrectionNotice-HID15ed.aspx. Please correct this error in all copies of the book and notify staff who may use this book.
