Site Description:

This is a rotation at Madsen FP, Sugarhouse FP, and Utah Diabetes Center with an emphasis on Diabetes Management (up to 3 pt visits/half day), and some Coumadin Telemanagement (approx 10pts/wk).

Rotation Description:

You will observe, participate and write up SOAP notes for diabetes pt management/education visits. You will call pts for Coumadin Telemanagement (approx 10/wk), provide drug info/answer questions for providers, develop therapeutic regimens, evaluate pt medication regimens, present 2 patient case presentations, attend 1 on 1 case /topic discussions, and give an inservice or create a pt education handout. You will be interacting extensively with both patients, providers and ancillary personnel.

Activities Evaluated:

Outcome R2: Provide evidence-based, patient-centered medication therapy management with interdisciplinary teams.

Readings and Preparatory Work:

The latest "Standards of Care" for Diabetes (found in the January supplement of Diabetes Care Journal each year), the latest JNC guidelines, and the latest NCEP guidelines.

Project/Inservice Description:

You will give a 20-30minute inservice to either the clinic ancillary personnel or providers OR you will create a 1-2 page patient handout on a given topic (i.e smoking cessation tips, "what patients need to know about statins", etc)
