Greenwood Health Center Rotation
(Primary Care with emphasis on Hyperlipidemia and Diabetes)

Description of Practice Site:
Greenwood Health Center is a primary care clinic that includes the following medical groups: Internal Medicine, Family Practice, Ob/Gyn, and Pediatrics. Clinic also houses an on-site pharmacy, optical center, lab, x-ray, and physical therapy department. Patients are referred to the clinical pharmacist by the various providers and are seen by appointment. Clinical pharmacy services currently offered include medication review and recommendations; medication management by collaborative practice in hyperlipidemia, hypertension, and diabetes; patient education and clinical pharmacy consultation in diabetes, hypertension, asthma, polypharmacy, herbal/natural medicines; and drug information.
Average Number of Patients:
3-5 patients seen per day, by appointment.
Patient Care Responsibilities in this Area:
The resident will manage lipid clinic, hypertension clinic, and diabetes clinic patients under collaborative practice agreement that includes developing goals of therapy, educating patients on therapeutic lifestyle changes, prescribing and adjusting medications as needed, checking vital signs, and ordering laboratory tests. The resident will also provide patient education on disease state management; evaluate drug therapy regimens and provide recommendations for optimizing drug therapy, monitoring, and patient compliance; document all patient visits in the electronic medical record; and bill for services under current University Health Care procedures. Resident may also be responsible for staffing in the pharmacy for a few hours a week, depending on resident preferences and/or pharmacy staffing needs.
Additional Resident Requirements During Rotation:
Resident will present an inservice topic or journal club. Resident will also be assigned projects that will contribute to patient care and the resident learning experience during the rotation.
