Sara Vazquez, PharmD
Clinical Pharmacist
Dr. Vazquez received her PharmD from the University of Tennessee in 2005 and completed a pharmacy practice residency with ambulatory care emphasis at the Regional Medical Center Memphis in 2006. She has been with the Thrombosis Service since 2006. She enjoys the multi-faceted nature of her practice in anticoagulation management, teaching/precepting, writing, and research. She is board-certified in pharmacotherapy and anticoagulation care. Her interest area is post-thrombotic syndrome.

Jolena Hagen, PharmD
Clinical Pharmacist
Jolena received her PharmD from North Dakota State University in 2003. After moving to Utah from cheesehead country of Green Bay, WI she accepted a position with the Redwood Anticoagulation Clinic and has been there since 2007. She enjoys working with students and has loved learning a new area of expertise in anticoagulation. She recently changed to part-time to enjoy some much needed time with her 1-year old baby girl, Addison.
For fun, Jolena loves to travel with her husband and Addison to see her family and friends (however has learned this is a whole new adventure with a baby), experiencing all that Utah has to offer, and run run run (after Addison and for fun). |
