Community Resources

Community resources are available in most communities across the nation. Larger cities generally have more established resources than smaller communities. Every community is different. Community resources can be essential components of a successful discharge plan. Your treatment team will work with you to determine what is available near your home and what you will need to have a successful discharge to home.

Below are listed some of the community resources available in the Salt Lake City area. These may provide a guideline that may help you find resources in your area as well.

Governors Council on Disabilities

The Rehabilitation Center finds the Governors Council to be a critical link for the disabled individual to learn essential new skills. The Governors Council works to create advocacy and leadership within the disabled community.

Intermountain Therapy Animals

Therapy animals are an important tool for some patients to re-connect to a meaningful life after a life-changing event. Intermountain Therapy Animals and the Rehabilitation Center for decades have partnered together to help patients move through this difficult transition.

International Paralympic Committee (IPC)

The Rehabilitation Center is fortunate to have 2 Paralympic medalists that work for the Rehab Center and 2 of our staff that volunteer for the IPC to provide more opportunities for persons with some level of impairment or disability to understand that they can engage in life-enriching activities, at the elite athlete level or at the casual activity level or any level in-between.

University of Utah Stroke Center

The Stroke Center has been a driving force in developing Utah's premier emergency and acute services, provider education and prevention programs. A multidisciplinary team of physicians work together to provide a wide range of diagnostic and therapeutic options for stroke patients.

The Stroke Center emphasizes acute intervention and is engaged in clinical trials with the goal of optimizing outcomes for stroke patients. The Rehabilitation Center works collaboratively with the Stroke Center to achieve the optimum patient outcomes for stroke survivors. Our work together includes the clinical continuum of care and research to improve stroke care in the future.
