The Outpatient Rehabilitation Program at Sugar House Family Health Center provides one or more rehabilitation services to individuals with a variety of disabling conditions. Therapy is designed to help those who have difficulty with such tasks as walking, speaking, remembering, personal care, returning to work/school, or getting around the community. The goal of treatment is to enable individuals to live independently within their own home and community.

- Spinal cord injuries
- Strokes
- Amputations
- General neuromuscular impairment
- Arthritis
- Cancer with physical impairment
- Parkinson's Disease
- Sports injuries
- Other musculo-skeletal pain/dysfunction
- Speech and language disorders
- General orthopedic conditions

- Patient & family education
- Cognitive rehabilitation
- Home based evaluation
- Community resource referrals
- Work/school re-entry (including on site evaluation and training)
- Driving evaluation and training
Admission Criteria
- Physician referral
- Medically stable
- Do not require hospitalization
- Financial resource identified
- Ability to participate and benefit from service
- Ability to make progress
