The Outpatient Rehabilitation Program at Sugar House Family Health Center provides one or more rehabilitation services to individuals with a variety of disabling conditions. Therapy is designed to help those who have difficulty with such tasks as walking, speaking, remembering, personal care, returning to work/school, or getting around the community. The goal of treatment is to enable individuals to live independently within their own home and community.

  • Strokes
  • Amputations
  • General neuromuscular impairment
  • Arthritis
  • Cancer with physical impairment
  • Parkinson's Disease
  • Sports injuries
  • Other musculo-skeletal pain/dysfunction
  • Speech and language disorders
  • General orthopedic conditions

  • Cognitive rehabilitation

  • Community resource referrals
  • Work/school re-entry (including on site evaluation and training)

Admission Criteria

  • Physician referral
  • Medically stable
  • Do not require hospitalization
  • Financial resource identified
  • Ability to participate and benefit from service
  • Ability to make progress
