Community Rehabilitation Services
The Rehabilitation Center at University Health Care is the only organization in the Intermountain West accredited by CARF for specialty programs under Home- and Community-Based Rehabilitation Programs. Our Home- and Community-Based Rehabilitation Program accreditation includes program accreditations for 4 programs: for Adults, for Adolescents, for Spinal Cord System of Care for Adults and Spinal Cord System of Care for Adolescents. These accreditations can ensure you, our patients and families that the care you receive through the Rehabilitation Center is amongst the best in the nation.
Community Rehabilitation Services provides a unique integrated approach to intensive rehabilitation in your own home and community. Therapists work with you to minimize barriers that affect real life activities, such as using public transportation, meal preparation, performing self care and daily living tasks, shopping, and money management. The rehab team guides you to develop the skills you need to return to school, work or other community activities.
Services Provided
Patients participating in the program receive therapy designed to maximize return of functional skill necessary for independent living, community access/reintegration and return to work, school or other productive activity. The Commission on Accreditation of Rehabilitation Facilities (CARF) guidelines are followed for treatment planning and interdisciplinary communication. Services include:
- Physical Therapy
- Occupational Therapy
- Speech Language Pathology
- Nursing/Case Management
- Functional Skills Coaching
- Work Re-entry
- School Re-entry
- Driving Evaluations
Services that are not included in the program rates, but may be negotiated on a fee-for-service basis include: splints, orthotics, durable medical equipment, counseling, neuropsychological evaluation, on-the-road driving testing, extensive vocational assessment, assistive technological services, supervision, home nursing, physician visits, lab fees, medications, etc.

Typical diagnoses may include, but are not limited to-
- Acquired Neurological Disease
- Brain Injury
- Stroke
- Spinal Cord Injury
- Multiple Trauma
- Debility as a Result of Medical Complications
Service Delivery Options
Service delivery is dependent on an individual's preferences and needs, including hourly, half day and whole day options. Financial arrangements are made on an individual basis.
