Heart Program Referral Process
If your doctor recommends that you undergo a heart transplant evaluation, he or she will either fill out the confidential referral form and e-mail it to us or contact our office directly by telephone. Your physician will send pertinent medical information regarding your condition. Once all information is received, the records will be reviewed by the transplant team. If it is determined that you could be a candidate for heart transplant, you will be scheduled for an initial clinic visit with the heart transplant team. Additional testing and appointments will be scheduled as needed for a full heart transplant evaluation. A full heart transplant evaluation requires approximately three days to complete. At the completion of the evaluation, a summary of all tests and a care plan will be shared with your referring physician.
