Kidney Transplant Program Referring A Patient

We accept referrals via phone (801) 581-2650. Our Renal Transplant Coordinator will help facilitate the initial evaluation.

To help expedite the referral process, please fill out the kidney patient referral form and fax it (801) 585-6373. Once a patient’s initial information is received, they will be mailed a kidney transplant information packet that contains a questionnaire and information on required tests. Once the questionnaire is returned and health maintenance tests are complete, the transplant center will request medical records and schedule the candidate for education and evaluation.
