For patients scheduling pre-travel counseling: Once you have your appointment scheduled, please fill out the online pre-travel questionnaire and bring your printed confirmation page with you to your appointment.

LDS Missionaries

Please fill out the online pre-travel questionnaire and bring your printed confirmation page, along with your mission booklet, to your appointment.

Post-Travel

For patients scheduling a Post-Travel appointment, please fill out the Post-Travel Form and bring it with you to your appointment, along with any related medical records.
