We have designed various financial plans to meet your individual needs. $Pricing GuidelinesThe Utah Center of Reproductive Medicine offers a full array of services to evaluate and plan your care and treatment. As specialists in Reproductive Endocrinology and Infertility, we see patients for various reasons and conditions. We are contracted with most major insurance companies for care such as new patient and follow up visits, evaluation, testing, diagnosis of infertility. Insurance companies we are contracted with include but are not limited to SelectHealth, Cigna, Altius, Regence Blue Cross Blue Shield, Coventry, DMBA, MultiPlan, PEHP, United Health Care, WISE, University of Utah Health Plan, and others to provide services that your insurance will cover.
In general, UCRM and the IVF and Andrology laboratories are not contracted with insurance companies for services related to fertility treatment, such as inseminations and in-vitro fertilization (IVF). This means that we are considered out of network for IVF and other fertility treatment benefits. If you have insurance coverage for IVF services, please let us know, as we can work with you and your insurance company to maximize your infertility benefits. Please note, that in many cases there will still be patient responsibility for a portion of the costs. We bundle our services into a global package so you know exactly what the treatment will cost. By purchasing a discounted package prior to the beginning of the cycle, patients avoid extra costs involved with paying for individual services.
UCRM does offer a couple different money back guarantee plans designed to provide qualifying patients seeking in-vitro fertilization (IVF) services a guarantee of a live birth or a partial refund if a live birth does not occur. We currently offer a "Single Cycle" money back guarantee plan and a “Multi Cycle” Money back guarantee plan.
Our Single Cycle plan includes upto one fresh IVF cycle attempt and one cryo (frozen) IVF cycle attempt within a one (1) year period for a set fee. The fee does not include medication costs, costs of other procedures or costs related to complications that may arise as a result of IVF treatment. The cost for this program is tiered and participation is extended after our committee reviews your case. The first step to participate in this guarantee program is to have an IVF consult, at which point you can discuss this option with your physician. You may also contact our program manager with any questions prior to your consult.
Our Multi Cycle plan includes upto four (4) attempts at IVF, with frozen attempts between each fresh attempt. Similar to the Single Cycle plan, the fee does not include medication costs, costs of other procedures or costs related to complications that may arise as a result of IVF treatment.
