Simple Summary

We hypothesize that carotid plaque inflammation is associated with local and systemic inflammatory pathways, and these may serve as future treatment targets. We also hypothesize that carotid plaque inflammation markers are elevated in recent ipsilateral ischemic stroke.

Inclusion Criteria

There are no differences between the University and VA populations that we will target for recruitment.
 
Surgical Subject Recruitment/Inclusion Criteria. Patients with carotid disease who qualify for carotid intervention (e.g. surgery) will be recruited for pre-intervention carotid MRI at two separate institutions, the University of Utah and the Salt Lake City VA medical center. Each year we routinely recruit 30 patients undergoing carotid surgery for preoperative MRI, well in excess of our recruitment goals of 50 patients for the 4 year NIH grant and 45 patients for the 3 year AHA grant. 
 

Exclusion Criteria

Exclusion Criteria. We will exclude patients with a gadolinium contrast allergy, eGFR<30 ml/min/1.73 m2. If the patient has an eGFR<30, they may still receive a noncontrast examination for the evaluation of MRI markers of plaque inflammation on noncontrast MRI, including intraplaque hemorrhage. Patients with other contraindications to MRI (e.g. orbital shrapnel, pacemaker) will be excluded from the study. Exclusions will be made on the basis of safety and study goals only, without restriction in regard to gender, race, age, and socioeconomic status. Noncarotid stroke sources will not be excluded but instead will be recorded as confounders.
