Brief Summary

Advanced Hemodynamics in Special Cardiovascular Populations is a prospective registry comprised of invasive hemodynamics and clinical data collected from specific cardiovascular populations whose disease processes, long term outcomes, and responses to treatment are incompletely understood. 

The objectives of the study are as follows:

  1. To better characterize hemodynamic features obtained by different forms of MCS (Mechanical Circulatory Support) and to better understand changes over time with durable forms of MCS. 
  2. To help select the type of MCS and duration of support that would provide the best chance of cardiac recovery in patients with heart failure.
  3. To help select the specific patients who would have the best chance of cardiac recovery with MCS and to provide clinicians with guidance as to when MCS is no longer required for a given patient. 
  4. To better understand the hemodynamic changes after TAVR (Transcatheter Aortic Valve Replacement) and to evaluate hemodynamic parameters that will predict outcomes and guide management for TAVR patients. 
  5. To better understand the hemodynamic abnormalities in HFrEF (Heart Failure with Reduced Ejection Fraction) and HFpEF (Heart Failure with Preserved Ejection Fraction) patients and evaluate parameters that may predict outcomes and guide treatment in this population. 
  6. To better understand right ventricular performance in Pulmonary Hypertension patients at baseline and in response to various treatments. To find parameters that may predict outcomes and guide treatment in this population. 

Inclusion Criteria

Inclusion criteria:

  1. Consenting adults (age > 18) with one of the following:
    1. Acute or chronic heart failure patients being considered for MCS.
    2. Acute or chronic heart failure patients who are being supported with temporary or chronic (durable) MCS.
    3. Acute or chronic heart failure patients with established or suspected HFrEF and/or HFpEF. 
    4. Patients with established or suspected pulmonary hypertension (WHO Groups 1 through 5).
    5. Patients who are undergoing TAVR.

Exclusion Criteria

Exclusion criteria:

  1. Neither patient nor patient representative understands spoken English.
  2. Patient  not willing to provide written consent for participation.
  3. Patients who have contraindications to invasive catheterization.
  4. Patients with mechanical aortic valve.
  5. LVAD patients with surgically sutured aortic valve.
  6. Pregnant women
