Brief Summary

Aim 1.  Create a clinical registry of participants with severe mood disorders.  Approximately 1,000 patients treated with electroconvulsive therapy (ECT) at the University of Utah will be enrolled in an observational study.

Aim 2.  Identify genetic variants associated with severe mood disorders.  Genetic and clinical data will be shared with the Psychiatric Genomics Consortium (PGC) in order to perform a genome-wide association study (GWAS) comparing cases of severe depression versus non-depressed controls.

Aim 3.  Identify genetic variants associated with clinical response to ECT.  A subset of participants with prospectively measured clinical outcome data will be analyzed by the PGC to perform a GWAS of clinical response to ECT.

Aim 4.  Discover novel biological, psychological, and social features that predict ECT response.  We will deeply and broadly phenotype complete responders and non-responders (n=100 per group) and compare participants that fall into these two extreme groups.

Inclusion Criteria

1. Age 18 or older
2. DSM-5 diagnosis of major depressive disorder or bipolar disorder
3. Current or past mood episode treated with ECT

Exclusion Criteria

1. Mood disorder due to head trauma
2. Diagnosis of a primary psychotic disorder (e.g., schizophrenia or schizoaffective disorder)
3. Currently unable to provide consent due to cognitive impairment, as deemed by the treating physician.
