This patient is a current student athlete at the U of U.
Follow the steps below to complete this registration:
1. Ask the patient: Is your visit in relation to an accident or injury that occurred while performing or practicing as a student athlete?
2. If the visit is in relation to performing in his/her sport, enter all personal insurance first, then list Sports Medicine University (SPT) as the payer of last resort. If the patient has Tricare or Medicaid, list these insurances as payer of last resort. Ask patient for referral.
3. If the visit is not in relation to performing in his/her sport, do not list Sports Medicine University (SPT) as a payer.
