    Dr. Bergen grew up in Ogden and Layton and currently lives in Farmington with his wife and 5 children. He is an identical twin and is married to an identical twin. He enjoys anything outdoors including pickleball, tennis, hiking and he loves spending time with his family and serving others.

    Nice person but he started my appt late. Seems good at what he does. On the last part of this question (Including you in decisions...) that's not really relevant to this part. There was only one decision -- to NOT go through the audiology test -- and that was denied. He said it's "mandatory" before ENT will see a patient.
