Patients may receive $20 gas cards as reimbursement for the cost of transportation to and from their appointment at Huntsman Cancer Institute. Patients who choose this option will be given the gas cards at their visit.

Patients may receive one $20 gift card for Lyft to transport them to and from their appointment at Huntsman Cancer Institute. Please note: The gift card may not cover the entire cost of travel. Patients must pay any remaining cost.

Patients who choose this service will be emailed a Lyft gift card. To use this service, patients must have a smartphone with the Lyft app installed and a second form of payment on file.

Patients may receive round-trip rides through Salt Lake Express Shuttle Service from Heber City and Vernal. Please be aware this service does not take patients directly to Huntsman Cancer Institute. Drop-off locations are downtown Salt Lake City (60 North Temple) or the Salt Lake City Airport. We can provide patients a $20 gift card for Lyft to transport patients to and from Huntsman Cancer Institute from the drop-off locations. See Ride Share Service for information.

Patients who choose this service will be emailed shuttle tickets and a Lyft gift card. Our transportation assistance coordinator will book the shuttle service for patients.

Each passenger is allowed one carry-on bag and a maximum of five checked bags. Fees are as follows:

• 1st bag: free
• 2nd bag: free
• 3rd, 4th, 5th bags: $10 each

Downtown Salt Lake City

• Pick-up/Drop-off location: 60 N Temple, West side of the building/Northeast corner of intersection, Salt Lake City, UT 84116
• Boarding instructions: Load on the west side of the Church of Jesus Christ of Latter-day Saints Conference Center, northeast corner of intersection.
• Daily departure times: 9:40 am and 5:10 pm

Salt Lake City Airport

• Pick-up/Drop-off location: 3920 W. Terminal Dr., Salt Lake City, UT 84122
• Boarding instructions: Go to the ground level outside of door L2. Shuttle will be between pillar 2B and 3B in the B lanes.
