No Show Policy

Missed Appointments

Due to the high number of patients requesting appointments, waiting times can be long. Because of this, the Pain Management Center has a low tolerance for missed appointments. This is our policy for patients who do not show up for their appointments.

You are a no-show if you:

  • Miss an appointment without telling us.
  • Miss an appointment with less than one day’s notice.
  • Miss an appointment because you have arrived 15 minutes late or later than the scheduled time.

If you must miss a scheduled appointment, please call our office the day before the appointment or three days before a new appointment. You can leave messages at all times including evenings and weekends.

If you no-show three times in a 12 month period, you may be discharged from the clinic.

New Appointment Rescheduling

For new appointments (evaluations), we request three days notice in order to have enough time to reschedule the appointment time. If you miss any new appointment (physician, physical therapy, or behavioral medicine) without any notice, we will be unable to reschedule your appointment.

Self Pay Patients

A self pay patient is any patient we are unable to verify insurance coverage. Self-pay patients must make a non-refundable deposit, which is approximately 50 percent of the total before we will schedule an appointment; in addition to your deposit, you will be billed for the

Deposit Rate for New Patient Visit

A deposit of $500.00 is required, $100 of which is non-refundable. This is for cancelled appointments (within two days) and not showing for appointments.

New Patient Visits

New patient visits include:

  • Physician appointment.
  • Physical therapy.
  • Behavioral medicine evaluation.
  • Treatment plan visit.
  • New patient orientation (no charge).
