Connect2Health is an organization working in medical clinics, which acts as a bridge between local community resources and under-served patients in need.

Purpose

Your service with Connect2Health is an opportunity to make a difference in your community by understanding social determinants of health and reducing health inequalities through advocacy and empowerment.

You will gain a meaningful volunteer experience in a clinical setting by interacting one-on-one with patients and their families. You will also collaborate with healthcare professionals and understand how clinical settings work.

Assigned site leader(s) and volunteer service coordinator

Days/Hours

Varying shifts Monday–Sunday (limited weekend shifts)

Minimum time commitment of one 3–4 hour shift per week and bi-monthly meetings for 2 consecutive semesters.

  • Summer Semester: March 15–April 15
  • Fall Semester: July 15–August 15

Qualifications

Open to all students and adults (must be 18 or over). Must commit to 2-consecutive semesters.

Volunteers should come with an open mind, willingness to learn, and must treat their peers, clinical partners, community members, and patients with respect.

Must maintain patient privacy and confidentiality at all times.

Duties/Responsibilities

  • Be a welcoming, compassionate ally and advocate for a patient and their family.
  • Arrive on time and stay for the entire duration of all assigned shifts.
  • Log hours for every shift using hospital kiosk.
  • Check personal and team shared emails. Follow instructions provided by site leader(s) and peer volunteers.
  • Check in with designated clinical staff. Introduce yourself as a C2H volunteer to the staff and patients.
  • Collect screening forms if applicable.
  • Meet with patients during their clinical visit.
  • Provide patient their personalized "Resource Prescription" created by online database.
  • Diligently document patient interaction on online database.
  • Notify clinical partners and site supervisors if you are unable to make your shift.
  • Attend bi-weekly team Reflection Sessions in addition to regular shifts.
  • Follow directions by clinical staff and volunteer services when requested.

Connect2Health volunteers never provide any medical advice or participate in any clinical procedures.

Do not enter patient rooms on precautions without training.
