very good experience

December 09, 2016

I really liked Jan Baker

November 27, 2016

I was very impressed and I really liked Jan Baker. She knew what she was talking about and listened to me.

October 13, 2016

Always a experience

October 07, 2016

I felt that I was not rushed through my appointment. This is an important factor for me choosing a provider.

October 06, 2016

Jan Baker is always reassuring, informative, and respectful. I *hate* that I am dealing with these problems, but I am very grateful that Jan exists.

September 13, 2016

I felt like I was in an assembly line and just being passed by because no one cared. The issues that I came in for were completely overlooked. I was not given a chance to even ask a question. Again it was like we don't have time for her so I'm just giving her the shaft.

September 12, 2016

So in both of my appointments with her she seemed annoyed with meeting to me. She made comments that seemed judgemental such as "My bad habits catching up with me." Here's the thing though, she never asked about any of my habits not sleep, not eating or ask about my schedule nothing like that. She also took a statement I said about not being in tune with my body and then linked it to my having been a trauma victim without knowing the rest of my history. Compared to the other people I have seen recently she seemed to make a lot of assumptions and jumped to a lot of conclusions based on my weight, the thumbnail sketch of my history. She did not do a good job of listening to me or addressing my concerns and her tone, her attitude and her approach came across as really critical of me as a person and not solution focused about my helath issues. And No I will not go back to her.

September 12, 2016

Jan Baker is awesome. I have never had a physician ever explain to me my condition and the way to combat the problem before. She was excellent!

August 28, 2016

This PA was not there to listen to me, she was in a hurry for me to hear what she had to say and to leave to her next appointment. Never to come back again.

August 28, 2016

very good

August 26, 2016

I was very impressed with Jan Baker. SHe is a very good listener..SHe asked me alot of questions .She explained my condition well so I would understand. I learned quite a bit.

August 19, 2016

great doctor

July 26, 2016

All aspects of the visit were well handled.

July 06, 2016

very hard to understand the answers to my questions. I would ask the questions again and still walked out of the facility confused as ever. not even sure what we were working on or what I was supposed to do regarding my problem. I felt that there was no resolve or questions answered as to why I was even there

June 25, 2016

I was given options and an opportunity to choose my treatment. I was assisted in getting the medication I needed at a reasonable cost.

June 16, 2016

I was surprised when I realized that Ms. Baker was upset with me from the get go. Aparently I had fail to schedule the follow ups I was supposed to have scheduled but I did not know about. it was the first time a health provider rants at me in such a way.

June 14, 2016

I will never go back to her, she did not listen to what I was telling her.

June 09, 2016

I appreciated that Jan was efficient and informative.

May 12, 2016

April 20, 2016

Yep I would recommend her

April 17, 2016

March 24, 2016

I was very impressed with Jan Baker. She helped me with my prolapse, finding a Pessary that works most of the time. She dispelled a myth told me by my physical therapist, which gives me confidence in my choice not to have surgery at this time.

February 20, 2016

She was great at what her specialty is I am sure, but I feel the error fell in scheduling, they placed me with this PA and she did a good job in her area of expertise.

February 10, 2016

very professional

January 15, 2016

Jan Baker, so very professional, thorough, listens, explains, and follow up. I am so impressed - been in healthcare research all my career and she exceeded my expectation.

November 26, 2015

My symptoms are still the same and the visit to the Uro/Gynecology Dept. did not help me.

November 24, 2015

She questioned me about being late when i told her that I had to leave to pick up my daughter at school. I wasn't late, however it took me a long time to check in bc there was only one receptionist and four people waiting at the desk in front of me. Seemed to be understaffed. I felt like the comment was a bit condescending and inappropriate. Very unprofessional behavior that I've never seen in a medical-care provider.

November 18, 2015

Jan Baker does a great job-is very knowledgable and helpful . I got a lot out of the appt. and am so glad that I came to see her.

October 25, 2015

Amazing results

October 14, 2015

Jan was very understanding and concerned about my issues. I feel very confident having her as my provider and I would definitely recommend her to others with the same condition.

September 21, 2015

September 21, 2015

The care provider was very knowledgeable, more so than any other doctor I went to for my problems.

August 24, 2015

Jan is the "bomb!" Very knowledgeable, professional, and easy to talk to.

August 21, 2015

August 21, 2015

August 20, 2015

Loved the fact the provider did not talk down to me.

August 06, 2015

very quick and efficient

July 19, 2015

Was not allowed to ask questions or speak of my issues. I made no decisions about my care and they dismissed any concerns I had about follow up treatment.

July 11, 2015

Provider was polite. She seemed knowledgeable about the issues. She seemed more intent on telling me what she wanted to get through rather than having any kind of discussion with me. When informing me about different options, she frequently suggested I google the options to find out any details.

June 29, 2015

very professional

June 23, 2015

June 15, 2015

Care provider was very knowledgeable about the care she was providing, but I did feel like she was talking down to me. Sometimes older patients are treated as if we are not capable of understanding or making decisions.

May 24, 2015

Excellent experience

May 19, 2015

May 01, 2015

April 11, 2015

April 09, 2015

I was late for the appointment. She was rushed. So the experience was compromised. My response to this survey might otherwise be a bit different.

April 04, 2015

Jan Baker is amazing, and i would absolutely recommend her.

March 27, 2015

It was rather short, but was as long as was needed to answer all my questions.

March 19, 2015

Comfortable with exam, Doctor was great helping me with what to do to hrlp usrlvf

March 18, 2015

Jan did a great job working with me. Pessary fitting is more of an art than a science and she has worked with me as I've come back to be fit with different pessaries when the previous one hasn't worked. As I've become more comfortable with my diagnosis and the (albeit pretty humiliating) experience of visiting the office with incontinence issues, my relationship with Jan has progressed. She's done a great job trying to get me fit properly.

March 09, 2015

#7 I'm confused a lot lately and things just don't stick in my mind....I'm not clear with things and it is not any one's fault. Jan Baker is extremely kind.

March 06, 2015

February 18, 2015

Jan Baker is the best . She was able to solve my long running problem with bladder infection.

February 16, 2015

The entire appointment was very positive from the time I entered the waiting room until I left. Everyone I came in contact with was very professional, yet also friendly and put one at ease. All my questions were answered fully and options and proceedures were explained to the max. Very impressed with Jan and her humaness and ability to listen to me.

February 15, 2015

Jan Baker is excellent!!!!

January 24, 2015

Like Ms. Baker a lot felt comfortable with her even though the problem is not comfortable to talk about.

January 24, 2015

NP Jan Baker care was outstanding. She took the time to tell me the amount of fluid I needed to take in on a daily basis. Limiting myself to this has improved the amount of time I sleep at night.

January 19, 2015

The doctor and assistant were very thorough and were not rushed. Took plenty of time to complete the test and give explanations.

January 19, 2015

Dr. Baker indicated that she would contact Dr. Dassel about my problem and get back to me. I am still waiting to hear back

November 20, 2014

Very nice experience and my daughter was pleased as well.

October 19, 2014

I was pleased that they could test the urine sample almost immediately and had results right away. I was very pleased with Jan in all aspects of clinic. I was pleased that she and her staff were able to accomplish so much and that she learned to check for a urinary infection. I was impressed with her knowledge, skill and professionalism.

October 11, 2014

September 29, 2014

I had a question about a medication when I got home, so I send her a message thru mychart, Jan got right back to me and answered my question it was great!

September 14, 2014

I have been struggling for almost a year and Jan Baker was more than helpful. Thank-you so much for her care & concern.

August 09, 2014

provider seemed like she was in a hurry and did not want to spend time going over my options with me and deciding together what would be the best options. she just decided what se was going to do and went ahead with it without really seeing what my thoughts or questions were with the plan.

August 04, 2014

June 19, 2014

She is always helpful.

June 18, 2014

I felt like both the fellow and MD both really listened to me and understood my frustration with my medical problem and care thusfar and my need for a long term care plan. Gave me a realistic plan to follow with very helpful printouts of my condition, additional meds that might help that I could order, etc.

June 07, 2014

May 15, 2014

very pleased with all the service

May 11, 2014

Advice to keep saying "no" when urinary urgency occurs was of little value. Chronic diarrhea was addressed at any time.

May 06, 2014

They seemed to have the answers but only after asking questions so that could evaluate how I felt and how I was feeling and evaluating my concerns.

April 05, 2014

I am genuinely impressed by her. She did an excellent job.

March 31, 2014

Jan Baker made me feel like a naughty little girl. I walked out of her office in tears.

March 28, 2014

I was very much impressed with both her knowledge and the caring way in which she presented it. She seemed to recognize that I respond to humor and used it to both of our advantages.

March 07, 2014

I've already recommended Jan to a friend.

February 22, 2014

Jan is an extremely knowledgeable and caring provider. She addresses all my concerns.

February 17, 2014

Dr Jan is one awesome lady-she explains what is being done-if changes need to be made as they were this time as to pessary size -she takes care of it explaining what to expect/if any problems do occur. :)

February 05, 2014

I was expecting a brief first visit with a follow up apt to try solutions. Instead, Jan spent a significant amount of time teaching and actually trying to solve my problem on the first visit. She was professional, kind, empathetic, and full of great info.

January 24, 2014

She was very considerate. She explained everything she did.

December 16, 2013

December 14, 2013

Baker was excellent.

December 14, 2013

Again, Jan was courteous, kind, professional, informative.

December 11, 2013

December 06, 2013

I thought Jan was very knowledgeable and helpful.

October 24, 2013

1-treatment included how I could/would use treatment. 2-explained why this treatment was good. 3-explained desired results, possible results, alternatives if first treatment wasn't successful 4-How,what when, why was both verbal and written.

October 17, 2013

I always have excellent care in Jan Baker's office.

October 16, 2013

It wasn't explained to me at the time of the visit that there can be adverse side effects from the pessary device. I was surprised to find this out later. Also, device doesn't fit well, is uncomfortable and painful, so I gave up wearing it after a week. So, I kind of feel like this was a waste of time.

October 14, 2013

Jan took the time to work on getting the prescription done as soon as possible to the Express Script Pharmacy. She has a great smile and always a happy person, enjoy the treatment she is able to give in a very professional manner, for a delicate subject. Definitely recommend her to those who need that type of care.

September 25, 2013

Jan was great. the receptionist over the phone was not helpful though

September 23, 2013

Jan Baker is extremely professional and has an excellent rapport with patients

August 13, 2013

We have been struggling with treatment because of all of my allergies to antibiotics. Jan decided that I was beyond her expertise so made an apt. for me to see the urologist. I appreciated all she has done to help me and to recognize when it was beyond her knowledge base.

August 07, 2013

She was in a hurry. It is a very busy office. I have had this same problem for a long, long time and I think they are basically sick of dealing with it but they seem to be the best option in the valley.

August 04, 2013

She was friendly and reassuring.

July 22, 2013

I had a hard time getting her to answer my questions. It may just be that she sees much worse than me in the clinic on a daily basis and didn't feel the need to go through everything with me because I'll be more healed soon, but I have no idea because I really don't know much more about my condition from meeting with her. Most of the useful information I've obtained has had to be from looking things up online. She was not rude, but was so quick to get me out the door, she dismissed questions and sent me on my way. Prolapse is a pretty devastating condition to deal with when a person is first diagnosed, which is why it's great when care providers take the time to listen and advise clearly on prognosis and the healing timeline.

July 17, 2013

I was extremely impressed by all of the people who treated me.

June 11, 2013

Even though we now are back in Arizona, really like the care and concern Dr. Jan Baker gives, trying very hard to be able to come back to Utah to continue under her care.

April 24, 2013

April 08, 2013

I requested more advanced information about different treatment options and expected to receive more advanced information about the physiology of pelvic floor disorders. I indicated that I had a grasp of medical terminology, and I was frustrated at the simplistic or dismissive answers (e.g "I'm not a surgeon" in response to "What is surgical treatment for prolapse?"). The information I was finally able to get was not at a high enough level for my understanding. I wished I had done more research in peer-reviewed journals and medical information web sites instead of depending on this visit for information and education. I recommend that Ms. Baker work on meeting patients where they are with different levels of information.

April 01, 2013

It was a good experience overall I just wish we would've gone more in depth with making sure to discover all the problems of the area of my body discussed

March 17, 2013

no medications were discussed

March 15, 2013

Entirely good experience.

March 11, 2013

I feel my care provider is very helpful and knows what she is doing.

December 17, 2012

December 14, 2012

She was good. She seemed on top of her game and I could be wrong, but I felt that she really didn't read through my chart before she came into the room. She was the 4th person I had seen for this problem and I felt like I was starting from the beginning, but maybe she did read the chart and just wanted me to repeat my story.

November 12, 2012

Jan was thorough, concerned, clearly an expert in her field. A positive refreshing change from many care providers
