Getting Ready for Your Appointment
Thank you for choosing University of Utah’s Department of Pediatrics for your child’s health care needs. We are excited to see you! Whether your appointment is in person or virtual, you will find helpful tips to ensure you get the most out of your visit.
Video Visits
Video visits are facilitated using the Amwell Now platform. Meeting link can be accessed on the web or through your mobile device with no downloading needed. Meeting invites are sent the day of the appointment. A medical assistant will contact you at the time of your visit to complete an intake and generate an invite for your appointment.
Please be advised, video visits are a medical appointment that require the patient to be present, to be seen, and heard in a private and safe location.
What’s Next?
You will receive an email from Ped Specialty Pre-Clinic to complete the following:
- Video Visits Sign Consent to Treat and HIPPA Upload your current insurance card.
- Complete the appointment health questionnaire.
Please take the time to complete these items before your appointment.
Quick Tips
To ensure a successful video visit, please follow these guidelines:
- Have the patient present use a private, well-lit space.
- Find a steady place to put your laptop, tablet, or phone.
- Avoid holding your device during the visit.
- Ensure your device is fully charged, or have it plugged in during the visit.
- Keep pets and other children in a separate room.
- Turn off anything that creates background noise and is distracting.
- Complete all pre-appointment paperwork through SignNow.
In-Person Visits
To ensure you get the most out of your visit with us, please do the following:
- Arrive on time.
- Bring your current Insurance Card(s), Photo ID, and a list of current medications.
- Create your personal MyHealth+ account and request proxy access before your appointment date. (See instructions above)
Get Connected With MyHealth+
MyHealth+ is an Intermountain Healthcare app that allows you to stay connected with your child’s health care team. Ask questions, request prescription refills and view pertinent medical records all at your convenience. We highly recommend you signing up and completing proxy access before your visit.
- If you are using a mobile device use the Apple or Google Play access buttons to download the MyHealth+ App. If you want to continue on the web, Click “On the Web” access button.
- Log in: If you already have a MyHealth account or a Select Health account, you can use the same username and password to sign in. Otherwise, click “Create Account”.
- Create your account, and you are in!
Request Proxy Access
You can have your child’s MyHealth+ account linked to your personal account by requesting proxy access. You can do that right in the app! Follow these steps to complete the request.
- Once logged in to MyHealth+, click on your name in the top left corner.
- Select “My Account” Get Connected with MyHealth+.
- Select “Individuals I Have Access To”.
- At the bottom of this list, click on “Request access to a minor” Your request is sent! Please allow 24–48 business hours for this to be approved. You will receive an email notification once that proxy access has been granted.
