What Is the Invisible Condition Alert Program?

Utah’s Invisible Condition Alert Program lets first responders know when a patient may struggle to communicate. The voluntary program allows you to ask to have a digital symbol placed on your vehicle registration to notify first responders of your condition. If you are a caregiver for a qualifying individual, you may also ask to have a digital symbol placed on a loved one's records. Early identification of an invisible condition allows first responders to approach situations with heightened awareness and sensitivity.

  • State ID card

Qualifying Medical Conditions

An invisible condition is a physical or mental condition that is not clear at first. There are several conditions that qualify for Utah’s Invisible Condition Alert Program:

How Do I Register for the Invisible Condition Alert Program?

A provider will need to certify you or your loved one's medical condition. The medical condition must meet the Invisible Condition Alert Program requirements in order to participate in the program. You can certify your condition with a variety of health care providers:

  • Licensed mental health providers

  • Nurse practitioner

  • Physician assistant

  • Physician

If you have visited your health care provider within the past year for the condition in which you want certified, please follow the below options to get your provider the appropriate form to review:

  1. MyChart: You can send a message to your provider using MyChart. When sending the message, please attach a signed copy of your form. Submitting your form through MyChart is the most efficient way for your provider to sign your form.

  2. Drop Off: You may deliver the completed and signed form to your provider's office. The clinic staff will then forward the form to your provider for review.

  3. Fax: You have the option to send a signed copy of the document to your provider via fax.

Once the form is received, your provider will assess your request and let you know if an appointment is necessary. Your health care provider must fill out paperwork for the Department of Motor Vehicles and the Driver License Division. When the Department of Motor Vehicles and Driver License Division receive the paperwork, a digital symbol will be added to your or your loved one's records. A letter will also be sent to the driver that includes a sticker to attach to the driver license or state identification card.

Do I Need to Renew My Status in Order to Remain a Participant in the Program? 

Once a completed form is added to your record, you will remain active in the program until you request to be removed. This applies to both the (DMV) Department of Motor Vehicle and (DLD) Driver’s License Division.

How Can I Be Removed From the Program?

If you no longer want to be involved in the program, fill out the Invisible Condition removal form(s) located at the bottom of the page. Your health care provider’s signature is not needed to complete a removal form.

What Form Do I Need to Fill Out if I Don’t Want a sticker Associated with My Vehicle?

You will need to fill out the Invisible Condition Request Form for the (DLD) Driver License Division.

Can I Request This Form on Behalf of a Family Member?

Yes, certain individuals who care for or live with someone with an invisible condition can submit a request form on the individual’s behalf. You can discuss your loved one’s eligibility with the health care provider treating their invisible condition. The individual with the invisible condition will need to provide consent unless they are a minor.

Can I Request This Form For a Minor?

Yes, parents and legal guardians can request forms for eligible minors. You can discuss your minor’s eligibility with the health care provider treating their invisible condition.  A sticker can be provided for a state ID if the minor doesn't have a driver's license.

How Do I Register My Home Address with the Program?

Once the Department of Motor Vehicles (DMV) and/or the Driver’s License Division (DLD) receive your completed form, they will update your driver's license record, which includes your home address.

Who Benefits From Enrolling in the Program?

Anyone living with, or transporting a person with, an invisible condition can benefit from this program. The goal of the program is to provide peace of mind to people with invisible conditions by promptly alerting first responders to their health care needs. First responders benefit from this program by knowing how to best respond to someone with an invisible condition. A family member or caretaker of someone with an invisible condition benefits from the program by knowing their loved one will be treated with understanding and respect.

Is This Form a Requirement for Me to Get a Driver’s License/ID or to Register a Vehicle?

No, this program is voluntary. The Invisible Condition Alert program allows interested individuals to disclose their condition on their record in order to provide a better understanding of any medical needs they may have by first responders. This program is also optional. Individuals with invisible conditions are not required to participate.

Will Completing an Invisible Condition Form Impact My Eligibility For a Driver’s License?

No, this program aims to raise awareness of your invisible condition for first responders. It does not affect your ability to get a driver's license. The DLD has separate requirements for medical conditions affecting driving safety.
