  • Dr. Eleazer is a native of South Carolina, attending the Medical University of South Carolina and completed his residency in internal medicine at the University of Kentucky Medical Center. After 3 years of practice in rural Kentucky, he returned to South Carolina and worked in a variety of settings before joining the faculty of the University of South Carolina School of Medicine. During his 25 years at the USCSOM, he founded the division of geriatrics, established the first geriatrics fellowship program in the state, directed a chronic care hospital and established a substantial research program. He served as Chief of Medicine at the Salt Lake City VA Medical Center and Associate Chair for Veterans Affairs for the Department of Internal Medicine at the University of Utah School of Medicine until 2019. He is currently the Acting Associate Director for Clinical Innovations at the Salt Lake City VA Medical Center GRECC [Geriatric Research, Education, and Clinical Center]. Dr. Eleazer reports he enjoys teaching medical students, residents, and fellows. He is currently the VA site principal investigator for the NIH-funded PREVENTABLE trial which is looking at whether statins can prevent dementia and functional decline in older adults. He is also the Principal Investigator for GHELP-TC, a clinical demonstration program designed to improve care transitions for older veterans who are hospitalized.
