From Crisis to Care: You matter outreach program
After a behavioral health crisis, the risk for harm is higher in the transitional days following discharge from an emergency department or inpatient setting. This time period can be difficult and overwhelming for individuals and their families. The You Matter Outreach program is dedicated to providing a safe and supportive transition back into the community.
Who we serve
Individuals aged 10 years and older who have recently discharged from a visit to a University of Utah Health hospital, emergency room, or crisis stabilization unit during which they expressed any level of suicidal thoughts or intentions
How our follow-up program can help
Significantly lower the rates of suicide, suicide attempts, and suicidal behaviors following an inpatient or emergency department discharge
Increase attendance in continuing mental health treatment to reduce the necessity of readmission
Ensure connection to additional support resources and assist with safety planning.
Program overview
Following an individual’s discharge from participating hospitals, four follow-up contacts are offered over the next 90 days. Our mental health professionals will call, email, or text at different intervals:
- 1-2 days,
- 7-10 days,
- 30-60 days,
- and 60-90 days after discharge
These contacts are intended to provide support, connection to resources, risk assessment, safety planning, and, if necessary, crisis intervention. Further follow-up contacts may occur as needed or requested.
For youth patients, follow-up calls are made to the parent or guardian; however, if the child is older than 12 years old, the parent or guardian may choose to grant permission for us to speak with their child directly.
You Matter Follow-Up Text Messages
As part of the You Matter outreach program, some patients may receive caring text messages after a recent visit or hospital stay.
These messages are meant to check in, remind you about follow-up care, and offer support or resources.
How It Works
- Text messages are sent only to patients who have agreed to receive texts from us.
- Messages are for support and information only—they are not used for emergencies or counseling.
- We never include details about your diagnosis, treatment, or medications in a text.
- You can opt out at any time by replying “STOP.”
