Recover at home after being treated at University of Utah Health with our Heal at Home program. We partner with Community Nursing Services (CNS) to provide care at home to patients all over Utah. CNS works with U of U Health to make sure you'll get the same expert care at home as you would in a hospital. 

Working together with multiple specialists, we’ll keep you connected to your provider and focused on your recovery. Our program reduces the chance of hospital readmission, decreases hospital stay time, and helps you save on health care costs. We’re here to give you the best chance at the best possible outcome, every step of the way. 

We focus on making recovery safe, comfortable, and effective by staying in close contact with your U of U Health care teams. All you need to do is stay home and focus on healing.

About Our Care Team

Specialists from U of U Health and CNS work together to make a care plan unique to you. You'll get similar care at home as you would in a hospital. Your U of U Health care team remains the same. CNS provides nurses, physical therapists, and occupational therapists based on your needs and on the plan developed by your care team.

Join the Heal at Home Program

Your surgeon or another care team member will discuss whether or not this program is a good fit for you during your preoperative visit. For programs where there is no planned procedure, we may discuss enrollment with you while you are in the hospital. 

What to Expect With Home-based Care

You are met by a home health nurse within hours of leaving the hospital. In some cases, their first visit might also be the day after your procedure. It depends on your needs. When your visits happen is decided by your supervising team. Nurses will check your vitals, review medications, and perform other nursing care in your home. Physical and occupational therapists will help you recover strength, stamina, and ensure your home environment is safe and accessible for you. You entire care team will communicate as needed with your care team at U of U Health.
