Is UCRM still part of the University of Utah?

While the clinic is no longer referred to as UCRM, the clinic maintains an academic affiliation with the University of Utah, including medical education, fellowship training, and research activities.

Appointments & Scheduling

How Do I Schedule an Appointment?

  • Call RMA Utah at 801-878-8888.
  • If you are new to fertility care or have not been seen by a UCRM provider within the past year, choose the new patient option.
  • Please leave a message if given the option to leave a voicemail. RMA Utah endeavors to return calls within 48 hours.

Appointment details are sent to the email on file until your Artemis patient portal is set up.

I am an established UCRM patient and am ready to move forward with treatment. What are the next steps?

UCRM patients who have been seen within the last 12 months will need a brief visit to establish care with RMA Utah. At your visit, your provider will update your medical record and transition your information to the new electronic medical record system. This ensures continuity and accuracy in your care plan.

If your last visit was more than 12 months ago, you will be scheduled as a new patient in the RMA Utah system. Your provider has access to all of your prior UCRM records for reference.

How do I log into the new patient portal, Artemis?

When you schedule an appointment, you will be sent login information for the RMA Utah patient portal, Artemis. The information will be sent to your email on file.

Where do I go for my appointments?

Patients now have access to three convenient locations to support their fertility care:

  1. Salt Lake City—675 Arapeen Drive, Suite 205
  2. Sandy—10150 Petunia Way  (egg retrievals, embryo transfers, sedation cases at this location only)
  3. Clearfield1725 East 1450 South

Can I still see an RMA Utah provider for gynecology or endocrine care not related to infertility?

RMA Utah has limited openings for non-fertility care. There may be a several month wait to get in. If you would like help in establishing with another provider at University of Utah Health, call women's health services at 801-213-2995.

I saw Molly Beck, Brenna Becker, or Anna Mair for gynecologic care. Can I schedule with them for non-fertility care?

Yes. Call U of U Health's women’s health main line at 801-213-2995 to make an appointment.

Questions About Refills & Prior Treatment

How do I get a medication refill?

In order to get a refill of a medication originally prescribed by a UCRM provider, you will need to establish care with RMA Utah.

If you are seeking refills for medication not related to fertility care and do not plan to establish care at RMA Utah, you have a few options:

  1. Ask your primary care provider to authorize the refill.

  2. If you need a refill for medication related to hormone replacement or endocrine needs, call the U of U Health main line at 801-213-2995. Advocates are available to help you get established with a women’s health provider for endocrine care.

Can I reach an RMA Utah provider through Epic?

No. With the transition to RMA Utah, all clinical care and questions are now managed through RMA Utah’s patient portal, Artemis. The RMA Utah providers are unable to access messages sent through Epic. Once you have been seen by an RMA Utah provider, primary communication will occur through Artemis.

Insurance & Billing

Did my coverage change?

RMA Utah participates with all insurance plans accepted at UCRM. However, individual benefits and authorizations may differ slightly and may require a new verification of benefits. As of April 20, 2026, care is billed under a new tax ID number. Because of this change, insurance coverage will be reverified. A financial coordinator will review your benefits and provide a clear estimate before treatment.

Does my previous self-pay quote from UCRM still apply?

After April 20, 2026, the University of Utah is no longer able to provide self-pay estimates. This process ended with the clinic’s closure. RMA Utah’s pricing may differ from UCRM. While we don’t have access to their specific rates, we understand this change may feel unexpected and we’re sorry for any stress or uncertainty it may cause. Our care team was only able to provide estimates for patients planning treatment within six months of April 20.

What insurance plans are accepted?

RMA Utah accepts a wide range of plans:

  • Aetna

  • Blue Cross Blue Shield

  • Cigna

  • UnitedHealthcare

  • Select Health

  • BridgeSpan Health

  • EMI Health

  • DMBA

  • First Health

  • MultiPlan

  • ValueCare

  • Healthwise

  • EBMS

  • Tall Tree

  • TRICARE

  • TriWest

  • University of Utah Health Plans

RMA Utah also accepts some employer-sponsored fertility benefits:

  • Progyny

  • Maven Clinic

  • Carrot

  • Kindbody

How are UCRM refunds or credits handled?

If you did not have an appointment scheduled prior to April 19, 2026, a refund was issued to you. If you had an appointment prior to the transition, the credit will be used to settle that balance first and then refunded to you.

Will I still receive statements from the University of Utah?

Yes, for outstanding balances and for any storage.

Who do I call for billing questions?

  • For care delivered prior to April 20, call University of Utah Fertility Billing at 801-213-3871.

  • For treatment after April 20, call RMA Utah at 801-878-8888.

Medical Records

Did my records automatically transfer?

If you were seen at UCRM between January 1 and March 31, 2026, your records were automatically transferred to RMA Utah through a secure, HIPAA-compliant agreement between University of Utah Health and RMA Utah.

If you were not seen during that time and would like to continue fertility care with RMA Utah, the care team at RMA Utah will assist you in completing any paperwork needed to access records of your prior treatment.

How can my records be sent to another provider?

The fastest and easiest way to transfer your medical records is for the new health care provider to fax a request on their letterhead to 801-581-2177.

When records are requested for continuing care, a signed patient authorization is not required under HIPAA. If the new provider cannot fax the request, you may complete a medical records authorization form available in MyChart or through U of U Health’s Medical Records Office. If you are having difficulty accessing your MyChart account, you can reach their help desk at 801-213-5555.

Specimens & Storage

Were my stored embryos, eggs, or sperm moved?

No, not without your written consent. Your specimens remain under the custodianship of University of Utah Health. They will continue to be monitored and maintained exactly as they are today, and storage billing will remain the same. If a transfer is required, your care team will guide you through the process and assist you with any necessary paperwork and next steps.

When requesting transfer of embryos, eggs, or sperm, all specimens must be moved together. This helps keep your care streamlined by avoiding multiple storage bills, ensures all specimens are stored safely in one location, and allows your care team to have access to all available specimens—including backups—during treatment. In some cases, specimens are stored together and cannot be separated safely.

How do I get testing of my stored embryos?

Schedule an appointment with an infertility specialist, either at RMA Utah, or the fertility clinic of your choice. All embryos will need to be transferred to that clinic where embryo biops(ies) will occur.

Who do I contact with questions about my stored specimens at University of Utah?

You may contact the laboratory directly at 801-581-3740.

Referring Providers

Does RMA Utah do IVF monitoring for outside fertility clinics?

Yes. Fax an order to 801-878-8890 with details of any lab work, ultrasounds, or other care that is being requested. Patient contact and billing information should be included with the request.

How Can I send a referral to RMA Utah?

RMA Utah is accepting referrals from community providers for fertility care. Send referral details including the referral reason, requested provider (if applicable), and any pertinent medical records along with complete demographic and insurance information to one of the contact options below:

  • Email—Utah_PatientServices@ivirma.com (preferred)
  • Fax—801-878-8890

University of Utah providers may place a referral in Epic as usual. The referral is FEMALE INFERTILITY REFERRAL PANEL and choose AMB Referral to Reproductive Endocrinology. This will default as an outgoing referral to RMA Utah.
