Now Taking Physician Referrals

New patient appointments require a referral from a patient’s primary doctor. Demographic information, such as the patient’s name, date of birth, home address, phone number, and insurance company should be included with the referral.

Records pertinent to the patient’s vasculitis history should be mailed or faxed to University of Utah Health prior to consultation (see address below). These records should include clinic notes, discharge summaries, diagnostic studies, radiographic studies, biopsy reports and treatment regimens. The patient’s primary doctor should include a one-page summary of the patient’s disease history and past treatment regimens. Specific questions that either the patient or the patient’s doctor would like addressed should be included in the one page summary.

CDs of imaging studies and glass slides of pathology specimens should be mailed to the Vasculitis Clinic (see address below) prior to consultation.
